It's in spanish but easy to fill out with very little knowledge of spanish. (I never took any spanish and can follow this easily)
Nombre - first name, Apellido - last name, Sexo - sex, Fecha de Nacimiento - date of birth, Dirección - address, Ciudad - city, Estado - state, Código Postal - zip code, Teléfono - telephone
Also you have to check off the gustaria recibir gratis de Tide box
